Polineuropatia simétrica distal no paciente diabético: estudo de prevalência e perfil epidemiológico em um ambulatório médico de Palhoça, SC

Objective Identify the prevalence of symmetrical distal polyneuroneuropathy in diabetic patients and epidemiological profile, in an outpatient clinic of medical specialties Methods The study included 137 patients, of both genders, aged 18 years or older, patients with diabetes in medical records. In these patients was applied the Michigan Neuropathy Classification Instrument (MNSI) and along that a visit to medical records to fill out clinical and epidemiological data on patients was made. The data were tabulated in Excel and analyzed in Software Statistical Package for the Social Sciences 18.0 (SPSS) Results The prevalence of symmetrical distal polyneuropathy was 38% (52 people). Among these 35 women (37.3%), 3 smokers (5,8%), 46 with high blood pressure (88.5%), 51 with type 2 diabetes (98.1%), 14 with a previous diagnosis of neuropathy (26.9% ), 14 with hypothyroidism (26.9%). Regarding the treatment in use: 39 oral antidiabetics (75%), 39 metformin (75%), 11 glibenclamide (21.2%), 6 other oral or injectable antidiabetics (11.5%) and 26 insulin (50% ). The mean age was 66.3 years, the mean time of diabetes was 14 years and the mean glycated hemoglobin was 7.8%. Conclusion The results presented show a prevalence of PNDS of 38% through the Michigan neuropathy classification instrument, most of which are unknown in this diagnosis. The majority were type 2 diabetics, hypertensive, obese and with more than 10 years of illness.
Objetivo: identificar a prevalência de polineuropatia distal simétrica em pacientes diabéticos e o seu perfil epidemiológico em um ambulatório de especialidades médicas. Métodos: foram incluídos na pesquisa 137 pacientes, de ambos os sexos, com 18 anos ou mais, cujo diagnóstico de diabetes constava em prontuário. A esses pacientes foi aplicado o Instrumento de Classificação de Neuropatia de Michigan (MNSI) e, em conjunto, houve a visitação aos seus prontuários para preenchimento de dados clínicos e epidemiológicos. Os dados foram tabulados no Excel e analisados no SPSS (Software Statistical Package for Social Sciences 18.0). Resultados: a prevalência de polineuropatia distal simétrica foi de 38% (52 pessoas). Entre essas, 35 eram mulheres (37,3%); 3, tabagistas (5,8%); 46 hipertensos (88,5%); 51 com diabetes tipo 2 (98,1%); 14 com diagnóstico prévio de neuropatia (26,9%) e 14 com hipotireoidismo (26,9%). Em relação ao tratamento, 39 utilizavam antidiabéticos orais (75%), 39, metformina (75%), 11, glibenclamida (21,2%), 6, outro antidiabético oral ou injetável (11,5%) e 26, insulina (50%). A média de idade foi de 66,3 anos; a média de tempo de diagnóstico, 14 anos, e a média de hemoglobina glicada foi de 7,8%. Conclusão: os resultados apresentados mostraram uma prevalência de polineuropatia distal simétrica de 38% através do Instrumento de Classificação de Neuropatia de Michigan, sendo que muitos deles desconheciam esse diagnóstico. A maioria era diabético tipo 2, hipertenso, obeso, e com mais de 10 anos de doença.
