Due to their effectiveness, antiresorptive agentes such as bisphosphonates and denosumabes are widely used. However, these drugs have been associated with the occurrence of osteonecrosis of the jaws (ONJ), called “medication-related ostenecrosis of jaw (MRONJ)”. The pathogenesis of maxillary osteonecrosis is not fully understood yet, however it’s known that among a variety of possible etiological factors, the inhibition of recruitment and the apoptosis of osteoclasts (essential in the reduction of bone reabsorption) caused by these drugs, becomes an obstacle when there’s a necessity of a normal alveolar regeneration, once these cells are fundamental elements in the first stages of it. The goal of this work is to revise the indications, means of action adverse effects of antiresorptives and to clarify the physiopathology, signs and symptoms, risk factors, and therapeutical protocol of these types of osteonecrosis.Antireabsortivos são potentes inibidores da reabsorção óssea empregados no tratamento da osteoporose e das alterações do metabolismo ósseo. Devido à sua eficácia, antirreabsortivos como bisfosfonatos e desonumabes são amplamente utilizados. Porém, esses medicamentos tem sido associados à ocorrência de osteonecrose dos maxilares(ONJ), denominada “medication-related osteonecrosis of jaw”(MRONJ). A etiopatogenia da osteonecrose dos maxilares não está completamente compreendida. No entanto, sabe-se que entre uma variedade de possíveis fatores etiológicos, a inibição do recrutamento e a apoptose dos osteoclastos causadas por estes fármacos, tornam-se um obstáculo quando existe necessidade de uma normal regeneração alveolar, por estas células serem elementos fundamentais nas primeiras fases desta.
