Resumo
A higiene bucal em pacientes críticos internados em hospitais muitas vezes é
negligenciada pela equipe multidisciplinar, muito pelo desconhecimento de sua importância e
relevância, bem como pela falta de um protocolo institucional e treinamentos da equipe por um
profissional habilitado. O objetivo desta revisão de literatura é estabelecer um protocolo de
higiene bucal destinado a pacientes internados em unidades de terapia intensiva para adultos,
com o propósito de destacar a relevância desse procedimento para a saúde do paciente, além de
padronizar os materiais utilizados e a maneira correta de executar a higiene bucal. Para a
presente pesquisa foram utilizadas as bases de dados Google acadêmico e National Library of
Medicine (Pubmed). Foi utilizado a pergunta de estudo “Protocolo de higiene bucal em UTI
adulto”. A efetividade, a frequência e o uso do digluconato de clorexidina a 0,12% e a utilização
ou não de métodos mecânicos de higiene bucal foram os principais pontos divergentes
encontrados nas literaturas revisadas. Os artigos com informações divergentes foram
devidamente avaliados e explicados, gerando assim um protocolo mais fidedigno e assertivo.
Foi estabelecido a frequência de higiene bucal a cada 12 horas respaldada pela eficácia
comprovada da propriedade de substantividade do digluconato de clorexidina a 0,12% e o passo
a passo do protocolo irá orientar de maneira concisa o leitor. No momento o digluconato de clorexidina a 0,12%, continua sendo considerado o padrão ouro na eleição para um método
químico, não substituindo a escovação dental, que é o método mecânico, mas sim
complementando um ao outro em sinergia para um objetivo comum que é a redução na taxa de
morbidade e mortalidade decorrente da pneumonia associada à ventilação mecânica (PAVM).
Oral hygiene in critically ill patients admitted to hospitals is often neglected by the multidisciplinary team, largely due to a lack of awareness regarding its importance and relevance, as well as the absence of an institutional protocol and team training by a qualified professional. The objective of this literature review is to establish an oral hygiene protocol for adult patients admitted to intensive care units, with the purpose of emphasizing the significance of this procedure for patient health, as well as standardizing the materials used and the correct way to perform oral hygiene. For the present research, the Google Scholar and National Library of Medicine (PubMed) databases were used. The research question "Oral hygiene protocol in adult ICU" was employed. Effectiveness, frequency, and the use of 0.12% chlorhexidine digluconate, as well as the utilization or non-utilization of mechanical methods of oral hygiene, were the main divergent points found in the reviewed literature. Articles with divergent information were thoroughly assessed and explained, thus generating a more reliable and assertive protocol. The frequency of oral hygiene was established at every 12 hours, supported by the proven efficacy of the substantivity property of 0.12% chlorhexidine digluconate, and the step-by-step protocol will guide the reader in a concise manner. Currently, 0.12% chlorhexidine digluconate, continues to be considered the gold standard for selecting a chemical method, not replacing dental brushing, which is the mechanical method, but rather complementing each other in synergy for a common goal: the reduction in morbidity and mortality rates resulting from ventilator-associated pneumonia (VAP).
