Resumo
Uma forma de prestação de serviços que vem se discernindo bastante nos
últimos anos diz respeito a Atenção Domiciliar (Home Care), onde seu principal
público são pacientes idosos, portadores de síndromes e patologias degenerativas
ou crônicas e pacientes em fase terminal. Os cuidados acontecem num ambiente
extra-hospitalar, ou seja, na residência e conta com uma equipe multidisciplinar que
oferece total suporte. A visita domiciliar pode ser entendida como o atendimento
realizado por um profissional ou equipe de saúde na residência do usuário, com a
finalidade de avaliar suas necessidades, de seus familiares e do ambiente onde se
encontra, a fim de estabelecer um plano assistencial voltado à recuperação ou
reabilitação. O principal objetivo deste trabalho é demonstrar, através do
levantamento de dados bibliográficos dos últimos 20 anos, a importância do
profissional farmacêutico na Atenção Domiciliar, dedicada aos pacientes geriatras,
em um ambiente extra-hospitalar, visto que é uma área da saúde ainda pouco
conhecida e em fase de crescimento em nosso país. Nesse contexto, o profissional
farmacêutico possui um olhar clínico e é de suma importância, uma vez que além
das atividades administrativas também desenvolve a responsabilidade técnica
voltada para o controle, distribuição e dispensação de medicamentos,
acompanhamento farmacoterapêutico voltado para a prescrição, onde verifica
possíveis problemas relacionados aos medicamentos, reações adversas, interações
medicamentosas e evolução do quadro clínico do paciente de acordo com a
farmacoterapia escolhida.

In this context, the pharmaceutical professional has a clinical perspective and is of paramount importance, since in addition to administrative activities, he also develops technical responsibility for the control, distribution and dispensation of medicines, pharmacotherapeutic monitoring focused on the prescription of medicines, where he verifies possible drug-related problems, adverse reactions, drug interactions and evolution of the patient's clinical condition according to the drug therapy chosen.
