A inserção da odontologia no ambiente hospitalar: relato de experiência

Resumo
O cirurgião-dentista é um profissional essencial na equipe interdisciplinar da atenção terciária, principalmente em Unidade de Terapia Intensiva. Além de cuidados bucais, esse profissional pode auxiliar no controle e na prevenção de infecções nosocomiais como a Pneumonia Associada à Ventilação Mecânica (PAVM). A microbiota bucal pode ser uma importante fonte de patógenos que contribuem para a infecção, portanto, medidas de higiene bucal são fundamentais na prevenção da PAVM, destacando-se a escovação dental e o enxaguante bucal com clorexidina como medidas eficazes na redução de patógenos orais e na prevenção da PAVM. A higiene bucal adequada e periódica é uma das medidas preventivas recomendadas para o controle da PAVM, e o cirurgião- dentista pode desempenhar funções importantes nesse sentido. Além disso, a remoção de focos infecciosos ativos e o tratamento e controle de alterações bucais que interfiram no internamento do paciente também são funções do cirurgião-dentista que atua no ambiente hospitalar. O objetivo deste estudo é descrever as experiências vivenciadas durante o estágio curricular do curso de Bacharelado em Odontologia, na área de Odontologia Hospitalar em dois hospitais públicos da região Sul do Brasil. As vivências das acadêmicas evidenciaram a importância de o cirurgião-dentista estar capacitado para identificar possíveis problemas na cavidade bucal que possam afetar a saúde do paciente internado, principalmente quando o mesmo faz uso de ventilação mecânica invasiva.
The dentist is an essential professional in the interdisciplinary team of tertiary care, especially in the Intensive Care Unit. In addition to bucal care, this professional can assist in the control and prevention of nosocomial infections such as Ventilator-Associated Pneumonia (VAP). The bucal microbiota can be an important source of pathogens that contribute to infection, therefore, bucal hygiene measures are essential in the prevention of VAP, with dental brushing and mouthwash with chlorhexidine being highlighted as effective measures in reducing bucal pathogens and preventing VAP. Adequate and periodic bucal hygiene is one of the recommended preventive measures for the control of VAP, and the dentist can play important roles in this regard. In addition, the removal of active infectious foci and the treatment and control of bucal alterations that interfere with patient hospitalization are also functions of the dentist working in the hospital environment. The objective of this study is to describe the experiences lived during the curricular internship of the Bachelor's Degree in Dentistry, in the field of Hospital Dentistry in two public hospitals in the South region of Brazil. The experiences of the students highlighted the importance of the dentist being trained to identify possible problems in the bucal cavity that may affect the health of the hospitalized patient, especially when the patient is under invasive mechanical ventilation.
