Introduction: Early mobilization has shown benefits in reducing the period of mechanical ventilation and improving functionality, including progressive therapeutic activities. The application of the cycle ergometer was mentioned as a viable alternative in the early intervention of critically ill patients, promoting the improvement of functional capacity, functional self-assessment and quadriceps muscle strength. Objective: To analyze the effects of early use of passive exercise associated with cycle ergometer in critically ill, mechanically ventilated patients. Methodology: Literature review carried out in scientific databases: PubMed, Virtual Health Library (VHL), LILACS and SciELO. Results and Discussion: 9 articles were selected to compose the analysis of the results. An improvement in the patients' strength, functionality and QoL was observed, although they still faced limitations at discharge, indicating the continued need for rehabilitation. The early introduction of passive exercise, such as cycle ergometer, showed potential to improve HGS. Therefore, combined physical therapy and exercise approaches, including the use of a cycle ergometer, may be beneficial for critically ill patients in the ICU. Conclusion: Early rehabilitation should be adopted on a large scale in the intensive care space, contributing to better patient outcomes. However, more rigorous and controlled studies are needed to confirm this assumption, analyzing with greater emphasis the use of passive exercise with a cycle ergometer in mechanically ventilated patients.
