Introduction: Orthognathic surgery is a procedure within the field of oral and maxillofacial surgery aimed at treating dentofacial deformities. In an effort to reduce complications and recovery time, the concept of minimally invasive orthognathic surgery has emerged. Its advantages range from minimizing tissue trauma to improving postoperative outcomes. Objective: To report and discuss less invasive approaches through the analysis of a case report. Case Description: A 22-year-old female patient sought a surgeon with aesthetic concerns about the appearance of her chin. Following a thorough history and examination of complementary tests, the patient was diagnosed with a combined skeletal Class III, mandibular prognathism, midface deficiency, and wide chin. The proposed treatment was early benefit orthosurgical intervention. Conclusion: Minimally invasive techniques in orthognathic surgery, when well-planned and executed, ensure satisfactory results with minimal harm to the patient.
