Resumo
A COVID-19 é uma infecção respiratória aguda causada pelo vírus da SARS-Cov-2 de grande transmissibilidade, contaminação assintomática, oligossintomática ou grave. Esse agente infeccioso tem capacidade de propagar formações de trombos, sendo capaz de aumentar a probabilidade de ocasionar o Infarto Agudo do Miocárdio . O presente artigo tem como objetivo correlacionar o agravamento do IAM com COVID-19, apresentar as principais complicações do IAM durante a pandemia e especificar os fatores de risco do infarto. Para escrever esse estudo foi utilizado como base metodológicas dos descritores “COVID-19 and Myocardial infarction”, “Complications and Myocardial Infarction” na base de dado PubMed, com critérios de inclusão artigos dos últimos 4 anos. Os casos de COVID-19 elevaram a troponina (principal marcador bioquímico para confirmar o infarto) e tem sido agravada com lesões miocárdicas e está nitidamente associada com o aumento da mortalidade. Grande parte dos pacientes afetados com maior gravidade apresentam fatores de risco cardiovasculares e doença cardiovascular prévia. Devido a ruptura de uma placa já presente, a tempestade de citocinas, injúria hipóxico (aumento da demanda metabólica e redução da reserva cardíaca), trombose, espasmo coronários ou lesão no endotélio.
COVID-19 is an acute respiratory infection caused by the highly transmissible SARS-Cov-2 virus, asymptomatic, oligosymptomatic or severe contamination. This infectious agent has the capacity to propagate thrombus formations, being able to increase the probability of causing Acute Myocardial Infarction. This article aims to correlate the worsening of AMI with COVID19, present the main complications of AMI during the pandemic and specify the risk factors for infarction. To write this study, the descriptors “COVID-19 and Myocardial infarction”, “Complications and Myocardial Infarction” in the PubMed database were used as a methodological basis, with inclusion criteria for articles from the last 4 years. Cases of COVID19 have elevated troponin (main iochemical marker to confirm infarction) and have been aggravated with myocardial injury and are clearly associated with increased mortality. Most of the more severely affected patients have cardiovascular risk factors and previous cardiovascular disease. Due to rupture of an already present plaque, cytokine storm, hypoxic injury (increased metabolic demand and reduced cardiac reserve), thrombosis, coronary spasm or endothelial injury.
