Introduction – Cervical cancer is a public health challenge and the third most common cancer in women. The Pap smear has revolutionized screening, but the pandemic has reduced the tests, impacting on prevention and treatment. Bringing significant consequences to oncological diagnoses and treatments. Objective – This study aims to analyze the impacts of the pandemic on execution of the Pap smear. Methods – The study is descriptive, retrospective and documental by analyzing cervical-vaginal exams from the Institute of Pathological Anatomy (IAP) from 2017 to 2023. It was divided into pre-pandemic, post-pandemic, pandemic and regular periods, grouping the exams by health insurance, SUS, private, normal cases, altered cases and age. The IAP, located in Sta. Bárbara d Oeste/SP, with units in Piracicaba, Americana and São Paulo, provided the data collected from the total volume of cervical-vaginal cytopathological exams. The data was collected anonymously, including information on volume of tests, source and age. Results – The decrease in Pap smears is worrying, impacting on the early detection of cervical cancer. In 2020-2021, there was an average reduction of approximately 15% on number of exams, followed by a gradual recovery in the following years, with an average increase of around 11.8%. The analysis indicates a significant impact of the pandemic, but the recovery trend is positive. About the altered exams by source of payment, there were notable variations, especially in SUS. The ‘Health Insurance’ category led the way in altered cases, suggesting greater availability and adherence to them. The 25-35 age group maintained a relatively high number of cases, possibly related to more attention in health care among this group. Conclusion – The data analyzed indicates an impact of the pandemic on number of Pap smears, evidencing a significant reduction. This decline reflects changes in the population’s preventive behavior during the challenging pandemic period.
