Eletroestimulação elétrica nervosa transcutânea como tratamento alternativo da neuropatia periférica induzida pela quimioterapia: uma revisão integrativa

Introdução: A Neuropatia Periférica Induzida por Quimioterapia (NPIQ) é frequentemente associada a diminuição ou atraso de doses ou cessação do tratamento neoadjuvante, com implicações diretas à eficácia terapêutica. No entanto, não existe padrão-ouro para profilaxia e tratamento. Entre os recursos terapêuticos disponíveis no manejo dos sintomas, a Estimulação Elétrica Nervosa Transcutânea (TENS) é uma estratégia promissora pela efetividade de baixo custo e disseminação. Objetivo: Investigar a viabilidade e eficácia do TENS no manejo dos sintomas da NPIQ em pacientes em tratamento neoadjuvante por meio de uma revisão bibliográfica integrativa. Método: Este estudo trata-se de um levantamento bibliográfico realizado por meio do método de revisão integrativa, utilizando as plataformas bibliográficas digitais PubMed, PEDro e Cochrane CENTRAL para coleta de dados, no qual, foram utilizados artigos científicos em inglês, publicados entre os anos de 2018-2023, obtidos através da pesquisa pelos termos em inglês: “TENS”, “Transcutaneous Electric Nerve Stimulation”, “electric stimulation therapy”, “cancer pain”, “neoadjuvant therapy”, “antineoplastic agents” e “CIPN”. Resultados: Foram analisados oito ensaios clínicos e quatro revisões sistemáticas que abordaram a utilização do TENS no manejo dos sintomas da NPIQ. A análise traz pontos consensuais como a melhora em sintomas álgicos e sensoriais; No entanto, revela uma importante falta de padronização dos métodos de seleção, medição e intervenção tal como pontos falhos no processo de avaliação. Conclusão: A literatura revela um consenso em relação a segurança, disponibilidade e facilidade de uso do TENS no manejo da NPIQ. Entretanto são escassos os estudos que possibilitem a sua aplicação como padrão de prática clínica.
Introduction: The Chemotherapy-induced Peripheral Neuropathy (CIPN) is often related to dose delaying, dose reduction or stopping neoadjuvant treatment, implying directly on therapeutic efficacy. However, there is no gold-standard for prophylaxis or treatment. Amidst the available therapeutic resources for symptoms management, Transcutaneous Electric Nerve Stimulation (TENS) is a promising strategy due to its low-cost effectiveness and dissemination. Objective: To assess the feasibility and efficacy of TENS use on the management of CIPN symptoms for patients under neoadjuvant treatment through an Integrative Literature Review. Method: This study is a bibliographic survey through integrative review method, using the digital bibliographic platforms PubMed, PEDro and Cochrane CENTRAL for data collection in which scientific articles were used in English, published between years 2018-2023 and retrieved by the use of the terms in English: “TENS”, “Transcutaneous Electric Nerve Stimulation”, “electric stimulation therapy”, “cancer pain”, “neoadjuvant therapy”, “antineoplastic agents” and “CIPN”. Results: Eight clinical trials and four systematic reviews were analyzed addressing TENS use on the management of CIPN symptoms. The analysis shows consensual points as improvements on pain scores and sensorial symptoms nonetheless it reveals an important lack of standards on selection, measurement, and intervention methods as well as failure points regarding the evaluation process. Conclusion: The literature reveals a consensus regarding safety, availability and ease-to-use of TENS on CIPN management. However, there is a paucity of studies to support its recommendation as a clinical practice standard.
