Resumo
Este relato de caso descreve o tratamento de uma lesão endo-periodontal em um paciente do sexo feminino, 18 anos, apresentando dor espontânea e usava aparelho ortodontico e após retirada da banda notou algo diferente e procurou atendimento. Durante o exame clínico e radiográfico, foi diagnosticada uma lesão endo-periodontal com envolvimento de furca e abcesso com secreção purulenta. O tratamento envolveu drenagem do abcesso, procedimentos endodônticos, incluindo: desinfecção e medicação intracanal com pasta de hidróxido de cálcio a 35% (UltraCal XS Ultradent) e, após 15 dias, houve retorno já sem abcesso, logo em seguida, foi feito o tratamento periodontal com raspagem e alisamento radicular. Após três meses, a paciente apresentou significativa cicatrização da lesão periodontal, confirmada por radiografias de controle. Este caso demonstra a importância de um diagnóstico preciso e uma abordagem integrada no manejo de lesões endo-periodontais, destacando a eficácia do tratamento combinado para a recuperação da saúde bucal do paciente.
This case report describes the treatment of an endo-periodontal lesion in a 18 year-old female patient presenting with spontaneous pain and wearing braces and, after removing an attachment of metal orthodontic band (tooth 36), noticed something different and searched for dental care. Clinical and radiographic examination diagnosed an endo-periodontal lesion with furcation involvement and abscess with purulent secretion. The treatment included abscess drein followed by endodontic procedures, such as: canal disinfection and intracanal medication with calcium hydroxide 35% (UltraCal XS Ultradent) and, after 15 days, the patient returned already without abscess, and started periodontal treatment with scaling and root planing. After three months, the patient showed significant reduction in tooth mobility and healing of the periodontal lesion, confirmed by follow-up radiographs. This case highlights the importance of accurate diagnosis and an integrated approach in managing endo-periodontal lesions, demonstrating the effectiveness of combined treatment for restoring the patient's oral health.
