INTRODUCTION: The anterior cruciate ligament (ACL) is a knee structure considered to be an important restrictor of anterior instability and internal rotation of the tibia. Rupture of this structure is the most common knee ligament injury, included only for complete ligament ruptures. The surgical technique of ACL reconstruction (ACLR) is essential for knee stability after injuries, but post-surgical recovery plays an equally crucial role. High-intensity treatment targets this recovery, improving knee functionality and ensuring lasting recovery after ACL surgery. METHODOLOGY: This is a narrative review in which the guiding question was: Is there therapeutic benefit in blood flow restriction training after ACL surgery. RESULTS: 22 studies were found, 12 in Pubmed and 10 in PeDro, with 1 duplicate removed. After reading the abstracts and title, 14 articles were excluded and 8 articles were included according to eligibility criteria. The selected articles were published between 2019 and 2023. DISCUSSION: The initial objective of this study was the effects of resistance training with restriction after ACL surgery. The results of the reviewed studies showed that exercises with blood flow restriction are efficient in improving quadriceps and hamstring muscle strength and physical function of the knee compared to exercises without blood flow restriction. CONCLUSION: Therefore, the present study showed that TRFS provides less overload on the knee joint, in addition to increasing strength and hypertrophy, promoting improvement in pain, physical function and quality of life.
