Atuação da fisioterapia pélvica na incontinência urinária e dor lombar durante a gestação

Resumo
A fisioterapia em obstetrícia surgiu com o objetivo principal de manter e promover uma melhor saúde física e emocional para as gestantes, realizando um trabalho com foco na prevenção de agravos que podem ocorrer durante a gestação e no pós-parto. Neste período os músculos do assoalho pélvico (MAP) podem apresentar diminuição da força devido aos processos fisiológicos e alterações anatômicas na pelve. Tem como objetivo descrever a atuação do fisioterapeuta durante o período gestacional, com ênfase na prevenção da incontinência urinaria (IU) e dor lombar. Revisão narrativa focada na abordagem fisioterapêutica em gestantes com lombalgia e/ou incontinência urinária, com o intuito de relacionar as informações adquiridas para a interpretação sobre o tema através dos descritores atuação da fisioterapia pélvica na gestação, pregnancy, physicaltherapy, pelvic floor, low back pain. Foram utilizados artigos e revisões de artigos encontrados nas bases de dados: Scielo, PEDro e Pubmed. As intervenções foram realizadas em mulheres saudáveis e sem complicações na gestação; os exercícios propostos incluíam treinos de força, exercícios de estabilização, TMAP, alongamentos e exercícios de consciência corporal que colaboraram para redução da lombalgia e IU. Este estudo possibilitou a análise da atuação da fisioterapia pélvica atuando no controle da dor lombar e da incontinência urinária em gestantes; entretanto ainda são necessários mais estudos com maior qualidade e rigor metodológico acerca dos tratamentos específicos para dor lombar em gestantes.
Physiotherapy in obstetrics emerged with the main objective of maintaining and promoting better physical and emotional health for pregnant women, carrying out work focused on the prevention of injuries that may occur during pregnancy and in the postpartum period (KEIL et. al., 2022). In this period, the pelvic floor muscles (PFM) may show a decrease in strength due to physiological processes and anatomical changes in the pelvis. Objective: To describe the role of the physiotherapist during the gestational period, with emphasis on the prevention of urinary incontinence (UI) and back pain. Methods: A narrative review focused on the physiotherapeutic approach in pregnant women with back pain and/or urinary incontinence, with the aim of relating the information acquired for the interpretation of the theme through the descriptors performance of pelvic physiotherapy in gestation, pregnancy, physicaltherapy, pelvic floor and back pain. Articles and reviews of articles found in the databases: Scielo, PEDro and Pubmed were used. Results: Interventions were performed on healthy women with no pregnancy complications; the proposed exercises included strength training, stabilization exercises, PFMT, stretching and body awareness exercises that collaborated to reduce the back pain and UI. Conclusion: This study made it possible to analyze the performance of pelvic physiotherapy in the control of back pain and urinary incontinence in pregnant women; however, further studies with higher quality and methodological rigor are still needed about specific treatments for back pain in pregnant women.
