Cuidados de enfermagem na manutenção do potencial doador de órgãos com Morte Encefálica: uma revisão integrativa da literatura

Introdução: A Morte Encefálica é definida como a perda total e definitiva das funções
do cérebro, que são determinadas pela interrupção das atribuições corticais e do
tronco encefálico. A manutenção do potencial doador é um desafio no cotidiano das
unidades de terapia intensiva. Objetivo: Identificar os principais cuidados de
enfermagem na manutenção dos potenciais doadores de órgãos em Morte Encefálica.
Método: Trata-se de uma revisão integrativa da literatura, a busca foi realizada nas
bases de dados Biblioteca Virtual em Saúde (BVS) e Pubmed. Foram identificados
140 artigos e aplicados critérios de inclusão e exclusão. A amostra final foi composta
por 5 artigos. Resultados: Os principais cuidados de enfermagem identificados nos
pacientes potenciais doadores de órgãos foram: manutenção da temperatura,
realização da dosagem de eletrólitos e da gasometria, realização da coleta de
hemocultura na abertura do protocolo de ME, utilização do cateter de Swan-ganz para
monitorização hemodinâmica, necessidade de controle da PAM, administração de
drogas vasoativas quando necessário, controle da glicemia e uso de antibioticoterapia
em caso de indicação clínica, manutenção das funções orgânicas, correção das
disfunções, correção do déficit de oxigenação, tratamento das alterações endócrinas,
renais e hepáticas, correção dos distúrbios de coagulação e correção de qualquer
outra alteração orgânica reversível. Conclusão: Foi possível identificar que os
cuidados de enfermagem nesta população apresentam relevância extrema, pois, a
partir deles o paciente se mantém estável para viabilidade da doação de órgãos.
Sugere-se a necessidade de capacitação constante para os profissionais visando a
manutenção efetiva do paciente potencial doador.
Brain Death is defined as the total and definitive loss of brain functions, which are determined by the interruption of cortical assignments and brainstem and to plan nursing care, it is necessary that the cause of death is well defined. To identify the main nursing care in the maintenance of potential organ donors in Brain Death. This is an integrative literature review, the search was carried out in the Virtual Health Library (VHL) and Pubmed databases. 140 articles were identified and inclusion and exclusion criteria were applied. The final sample consisted of 5 articles. The main nursing care identified in potential organ donor patients were: temperature maintenance, electrolyte and gasometry measurements, blood culture collection at the opening of the BD protocol, use of the Swan-ganz catheter for hemodynamic monitoring, need for MAP control, administration of vasoactive drugs when necessary, blood glucose control and use of antibiotic therapy in case of clinical indication, maintenance of organic functions, correction of dysfunctions, correction of oxygenation deficit, treatment of endocrine, renal and hepatic alterations, correction of coagulation disorders and correction of any other reversible organic alteration. It was possible to identify that nursing care in this population is extremely relevant, because from them the patient remains stable for the viability of organ donation. It is suggested the need for constant training for professionals aiming at the effective maintenance of the potential donor patient.
