Os efeitos da pandemia de covid-19 na investigação diagnóstica do câncer de mama em Santa Catarina, 2017-2022

Resumo
Analisar a tendência temporal dos efeitos da pandemia por COVID-19 na investigação diagnóstica
do câncer (CA) de mama em Santa Catarina (SC), no período de 2017 a 2022. Estudo ecológico de
perfil e séries temporais. Dados obtidos do Sistema de Informação do Câncer - SISCAN
disponibilizado pelo Departamento de Informática do Ministério da Saúde. Foram incluídos registros
de exames histológicos de mama de mulheres e homens do Estado de SC, com idade >20 anos, no
período de 2017 a 2022. A extração dos dados foi realizada por meio do TABWIN. Calculadas as
proporções de exames histopatológicos de mama, graus histológicos (I, II e III) e tamanho da lesão
(<2, 2 a 5, >5 a 10, > 10 e tamanho não palpável de CA de mama segundo faixa etária. Realizada
análise estatística por regressão linear simples, p<0,05. Encontrado 14.124 exames histopatológicos
de mama, homens (1,19%) e mulheres (98,1%). Revelou-se tendência temporal aumentada na
realização de exames histológicos de mama, com queda em 2020 devido à pandemia e com
recuperação em 2021. Maior proporção de lesões de grau I em mulheres de 20-39 anos, redução de
lesões de grau III. Mulheres de 40-59 aumento de lesões maiores (5-10cm) sugerindo possível atraso
no rastreio. Mulheres com >60 anos observou-se lesões >10cm, apontando rastreio tardio durante a
pandemia. A pandemia de COVID-19 impactou negativamente a detecção do câncer de mama, mas
a recuperação observada em 2021 destaca a resiliência dos serviços de saúde em SC.
To analyze the temporal trend of the effects of the COVID-19 pandemic on breast cancer (BC) diagnostic investigations in Santa Catarina (SC), from 2017 to 2022. This is an ecological study with a profile and time series design. Data was obtained from the Cancer Information System made available by the Department of Informatics of the Ministry of Health. Records of breast histological exams of women and men from the State of SC were included, aged >20 years, from 2017 to 2022. Data extraction was performed using TABWIN. Proportions of breast histopathological exams, histological grades (I, II, and III), and lesion sizes (<2 cm, 2 to 5 cm, >5 to 10 cm, >10 cm, and non-palpable size) of breast cancer were calculated according to age group. Statistical analysis was performed using simple linear regression, p<0.05. A total of 14,124 breast histopathological exams were found, with men accounting for 1.19% and women 98.1%. An increased temporal trend in performing breast histological exams was revealed, with a drop in 2020 due to the pandemic and a recovery in 2021. A higher proportion of grade I lesions was found in women aged 20-39 years, with a reduction in grade III lesions. Women aged 40- 59 showed an increase in larger lesions (5-10 cm), suggesting possible delays in screening. Women aged >60 years had lesions >10 cm, indicating delayed screening during the pandemic. The COVID-19 pandemic negatively impacted breast cancer detection, but the recovery observed in 2021 highlights the resilience of health services in SC
