Influência de diferentes técnicas obturadoras no preenchimento dos canais secundários

The premolars present morphological complexity of the canals. Studies indicate the prevalence of secondary canals to be found in the middle and apical thirds. The objective of the present study is to evaluate the influence of different shaping and filling techniques in the filling of secondary canals through two differents filling and shaping techniques. Forty artificial, radiopaque and unirradicular premolars had their crowns sectioned and had a standardized length of 19 mm length. Two secondary canals were made in the apical and middle thirds, in the mesial and distal walls of all specimens with a 0.4 mm diameter drill, and then the teeth were divided into groups according to the shaping and filling technique: MM/TCL- shaping with manual technique and filling with the lateral condensation technique, PT/TCL- shaping with the mechanized technique (pro taper system) and filling with the lateral condensation technique, MM/THT- shaping with manual technique and filling with tagger hybrid technique and PT/THT- shaping with mechanized technique (pro taper system) and filling with tagger hybrid technique. All specimens were x-rayed, and then the images were analyzed by a radiologist who did the evaluation blind. The evaluation crieteria were: “completely filled” for when there was radiopaque material throughout the extent of the secondary artificial canals, “partially filled” for when presented radiopaque material in part of the canals extension and “empty” for when it didn’t presente radiopaque material in your extension. After the evaluation, the chi-square test and the Pearson's test were applied to verify possible differences and where they were to fill the secondary canals. It was considered statistically significant for 0.05. The results showed a significant difference in the apical third, in which the PT/TCL group had the highest percentage of fully filled secondary canals while the MM/TCL group had the highest amount of empty canals. In the treatment of teeth where the prevalence of secondary canals in the apical third is larger, the lateral condensation technique presented a better performance when associated with the mechanized modeling technique.
Os pré-molares apresentam uma complexidade morfológica dos canais. Estudos indicam a prevalência de canais secundários encontrarem-se nos terços médio e apical. O objetivo do presente estudo foi avaliar radiograficamente a influência de diferentes técnicas de modelagem e obturação no preenchimento dos canais secundários por meio de duas técnicas de obturação e modelagem diferentes. Quarenta pré-molares artificiais, radiopacos e unirradiculares, tiveram suas coroas seccionadas e tiveram o comprimento padronizado em 19 mm. Foram confeccionados dois canais secundários nos terços apical e médio, nas paredes mesial e distal de todos os espécimes com uma broca com 0,4 mm de diâmetro e, em seguida, os dentes foram divididos em grupos de acordo com a técnica de modelagem e obturação: MM/TCL-modelagem com a técnica manual e obturação com a técnica de Condensação Lateral, PT/TCL- modelagem com a técnica mecanizada (Sistema Pro Taper) e obturação com a técnica de Condensação Lateral, MM/THT- modelagem com a técnica manual e obturação com a técnica Híbrida de Tagger e PT/THT modelagem com a técnica mecanizada (Sistema Pro Taper) e obturação com a técnica Híbrida de Tagger. Todas os espécimes foram radiografados e, em seguida, as imagens foram analisadas por um radiologista que fez a avaliação às cegas. Os critérios de avaliação eram: “Totalmente preenchido” para quando houvesse material radiopaco em toda a extensão dos canais secundários artificiais, “Parcialmente preenchido” para quando apresentasse material radiopaco em parte da extensão dos canais e “Vazio” para quando não apresentasse material radiopaco em sua extensão. Após a avaliação, foram aplicados os testes de qui quadrado e Teste de Pearson para verificação de possíveis diferenças e onde encontravam-se quanto ao preenchimento dos canais secundários. Foi considerado estatisticamente significante para 0,05. Os resultados mostraram diferença significativa no terço apical, na qual o grupo PT/TCL apresentou a maior porcentagem de canais secundários totalmente preenchidos enquanto o grupo MM/TCL apresentou a maior quantidade de canais vazios. No tratamento de dentes onde a prevalência de canais secundários no terço apical é maior, a técnica de condensação lateral apresentou um melhor desempenho quando associada à técnica de modelagem mecanizada.
