Monografia Acesso aberto Análise da segurança operacional na instrução de voo e a abordagem pelo Aeroclube de Bragança Paulista(2017) Sousa, Jefferson JoséThis research deals with flight safety at the primary moment of a pilot's trajectory, an instruction phase. Pretending the risks in the stage and the form of the theme during the training period. It also seeks the contributory factors for the occurrence of aeronautical accidents and incidents in the instruction phase and resources to improve flight safety. The Aeroclube of Bragança Paulista was mentioned in the survey and the accidents that occurred with the institution served as a basis for the study. A methodology adopted with bibliographical and documentary research, searching for theoretical references and identification in the legislation of the requirements for a pilot qualification grant and a capacity of the license teaching institutions aware of the importance of flight safety. An analysis of the data was done through graphs and charts, analyzed according to the theoretical basis. Characterized as an exploratory research with documents and bibliographical findings through the use of books, manuals, regulations and theses related to accidents and safety in hems operations. The approach used was qualitative. By analyzing data from international and national studies related to hems operations it is concluded that a significant number of accidents occur on landing or takeoff procedures, in addition to ground maneuvers, to the not prepared landing zones. The knowledge of aerodynamics of rotorcraft, together with the standardized training of communication between medical staff and pilot are essential for the accomplishment of missions without compromising safety beyond acceptable levels. The ems helicopter is a safe and essential life support tool but the urgent characteristic of the air medical flight must be taken into account during the training of teams to avoid jeopardizing the integrity of all on board due to the impulse to accomplish the mission.Monografia Acesso aberto Aspectos inerentes ao fator humano e suas interferências para a disciplina do voo(2019) Querino, Ralyson Benício SilvaEsta pesquisa teve como objetivo geral identificar como os aspectos inerentes ao fator humano podem influenciar na disciplina do voo. Caracteriza-se como uma pesquisa exploratória com procedimento bibliográfico e documental, em que foram utilizados livros, artigos científicos e documentários sobre o acidente envolvendo o voo 3142 da companhia aérea LAPA, em 1999, na Argentina. A análise de dados foi qualitativa, assim, os dados foram reunidos e analisados, de modo a oferecer uma compreensão mais aprofundada sobre a importância da disciplina do voo. Com este estudo, concluiu-se que o fator humano é fundamental para a segurança do voo, pois são principais responsáveis pelas operações a bordo, desse modo, qualquer ato de indisciplina como esquecer de realizar um procedimento ou realizar ações baseadas em “achismos” são atos de indisciplina que podem ser o estopim para um acidente aéreo. Porém, como visto, a indisciplina não acontece por um fator isolado, ou seja, muitas vezes, a própria companhia aérea apresenta falhas na disciplina organizacional, o que pode interferir para a disciplina do voo dos tripulantes. Inclusive, esta correlação foi apresentada no estudo de caso do acidente com o voo LAPA 3142, em que tantos fatores de indisciplina organizacionais como individuais contribuíram para o acidente aéreo que levou a 65 mortes.Monografia Acesso embargado Até que ponto a automação contribui para a segurança de voo? Data analysis was performed through documents and studies carried out on the aeronautics sector, according to the theoretical fundamentals, as well as accident investigation reports. Its main specific objectives were: to describe how the selection and training process of Brazilian air traffic controllers is performed, to compare the curricula of training courses, to define how English proficiency is determined, and to relate it to performance failures of air traffic controllers, and to demonstrate the changes in air traffic controller training to prevent and mitigate aircraft accidents. This study is characterized as a descriptive research, with a qualitative approach and bibliographical and documentary procedures using books, articles, monographs, dissertations, theses, reports, Manuals and Instructions of the Aeronautical Command, regulations, electronic sites and websites. At the end of this research, it was verified that the qualification of air traffic controllers in Brazil occurs at two institutions: at the Aeronautical Experts School and at the Air Space Control Institute. Both are military, yet independent, and with defined target audiences. The results indicate that there is no establishment of patterns in training courses, starting the selection process with different requirements for English proficiency, despite the fact that courses follow a minimal curriculum. It was observed that aircraft accidents involving air traffic controllers had, as a contributing factor, failures in communication between air traffic controllers and pilots due to insufficient aviation English proficiency. It was also concluded that there is a discerning treatment, dividing air traffic controllers between the ones who will follow military career, and the ones who will serve as voluntary civilians in temporary military service.Monografia Acesso aberto Excesso de jornada de trabalho e suas consequências na aviação executiva no Brasil(2021-11-09) Rochemback, LeandroA tecnologia tem literalmente voado graças aos estudos; atualmente temos aviões com uma tecnologia de ponta em todos os sentidos, telas e motores de última geração certificados com sistema de pouso automático e somado a tudo isso, espaços aéreos monitorados com radares de alto desempenho. Porém a cada novo acidente aéreo a pergunta é a mesma: por que que as tragédias acontecem na aviação? A população procura entender as razões que permitiram que tal evento acontecesse. Atualmente a ameaça de segurança mais comum decorre da realidade do desempenho humano abaixo do ideal. Este trabalho tem como objetivo mostrar e conscientizar que a fadiga humana e o excesso de jornada por parte dos pilotos da aviação executiva podem causar acidentes aeronáuticos fatais, desta forma o trabalho relata previamente alguns acidentes do passado através dos relatórios finais retirados do órgão CENIPA, que identificaram excesso de carga de trabalho e jornada por parte dos tripulantes além de violações das leis trabalhistas por parte dos proprietários de aeronaves. However, aeronautical accidents drew attention due to their fatality, so that it is possible to highlight the workload as a motivating factor. Thus, the present study aimed to analyze how the workload in the flight cabin can influence the occurrence of aeronautical accidents. For that, bibliographic research was used with the use of books and other scientific works that approach the theme in question. It is important to highlight that the automated system, related to flight management and control, enabled a safer and more efficient operation in the field of operational safety, especially flight cabins. However, professionals related to aeronautics are subjected to moments of stress and fatigue in the work environment, which cause various organizational problems and end up compromising the health of the worker due to the imbalance in the man-machine relationship. It also aims to encourage and adopt some practices to prevent future accidents in the aeronautical environment. The present research is characterized as explanatory, with a bibliographic and documentary procedure and with a qualitative approach. The analysis of the data was done through graphs and reports, analyzed according to the theoretical basis. At the end of the research, it is concluded that the main factors that contribute to aeronautical accidents are: pilot judgment, management supervision and application of the commands, arriving at the conclusion that it is necessary both in training and when already working, that pilots seek to improve their knowledge of Operational Safety, having to make correct use of the Checklist and maintain good conduct in the cockpit putting into practice everything about the CRM. Ignorance and non-observance of safety limits make human factors a major source of contribution to the occurrence of aviation accidents and incidents in civil aviation.Monografia Acesso aberto Gerenciamento do risco na aviação agrícola nos estados do Mato Grosso e Rio Grande do Sul(2016) Ciocheta, Rafael Da SilveiraThis study aims to identify the four most frequent aircraft occurrence categories and its contributing factors relating to agricultural aviation in the states of Mato Grosso and Rio Grande do Sul between the years 2013 and 2015, proposing ways that can be adopted to mitigation them. The choice of these two states is given by the national relevance in this type of operation, comprising almost half of the brazilian fleet of crop dusters. It is characterized as an explanatory research with bibliographic and documentary procedure through books, manuals, regulations and aircraft accident reports. The approach used was qualitative and quantitative. By analyzing the official statistics, it is concluded that the four occurrence categories identified as most frequent were the Loss of Control – Inflight, Loss of Control – Ground, Low Altitude Operations and System/Component Failure or Malfunction – Powerplant. It is characterized as an exploratory analysis, with bibliographic and documentary procedure by books, manuals, regulations and aircraft accident reports. It has been used a qualitative and quantitative approach. Through the official data analysis, it has been concluded that the use of aeronautical non-standard aeronautical phraseology stands for a contributing factor in many of known Brazilian and international aviation accidents and still reveals itself as a big villain when used indiscriminately. The purpose of the present paper is to present the current Brazilian research scenario regarding aeronautical accidents, demonstrating the shortcomings contained in the Organizational Factor used, as well as suggesting solutions to the problem. It is characterized as a research developed based on bibliographical studies the digital and print collection, historical analysis comparative with the methodology and applied standards in the investigations to accidents, aeronautical incidents and occurrences in ground occurred abroad, as well as interview with three professionals specialized in the aerial The research has a qualitative character, that is, it seeks to evaluate the degree of efficiency of the methodology used in the investigations. At the end of the research, it is concluded that the preventive methodology currently applied to aeronautical accident investigations in Brazil demonstrates a low level of efficiency of the public agencies in the search for the determining causes of the aeronautical accident, putting at risk the safety of the flight. The work awakens the critical sense of the members of the aeronautical community regarding the current Brazilian investigative procedure, allowing the discussion about the degree of efficiency of the related Organizational Factors.Monografia Acesso aberto Negligência no planejamento de voo: condições meteorológicas adversas na aviação geral no Brasil(2021-06-23) Rocha, Willy Manoel TirloniEste trabalho teve como objetivo demostrar que o conhecimento meteorológico aeronáutico é de suma importância para a segurança operacional não só na aviação de transporte regular de passageiros, mas também na aviação executiva, conhecida como aviação geral, chamando assim a atenção da comunidade aeronáutica quanto à importância do planejamento meteorológico adequado como também as consultas às publicações oficiais de meteorologia e o uso adequado de equipamentos embarcados a bordo das aeronaves para evitar a entrada em condições adversas de tempo. A pesquisa caracteriza-se como descritiva, tendo em vista que foram descritas as ferramentas utilizadas para um bom planejamento meteorológico antes a execução de um voo, utilizando o procedimento bibliográfico e documental a consultas de livros, artigos, reportagens e publicações oficiais. A abordagem utilizada foi qualitativa, analisando assim documentos dos acidentes e incidentes aeronáuticos através do portal oficial do CENIPA e da Agencia Nacional de Aviação Civil (ANAC). The approach used was qualitative and quantitative. The analysis of the data was done through summaries, graphs and tables, analyzed according to the theoretical basis. At the end of the research, it was concluded that single-pilot operation would not be as safe as dual-pilot operation in regular passenger transport, even in the face of the high automation systems available on aircraft. In relation to the methodology used, the present research is characterized as descriptive, with bibliographic and documentary procedure, with both qualitative and quantitative approach. For the analysis of the data, surveys were made in bibliographies and documents made available by several organs, such as, for example, ANAC, CENIPA, ICAO, links of SIPAER. At the end of the research, it is concluded that improving the quality of education requires a higher level of knowledge and that investing in the training of flight instructors will have repercussions on the training of the entire aviation workforce. Therefore, requiring higher education in Aeronautical Sciences, Civil Aviation or another related higher education, added to a postgraduate related to pedagogy, will bring a significant improvement to the aeronautical safety culture and will decrease the number of occurrences in the aviation of instruction and / or contributing factors arising from the instruction.Monografia Acesso aberto Proficiência inglesa na aviação: uma análise de erros comunicativos fatais e medidas de prevenção de acidentes(2019) Diniz, JulianaThis study analyzed how the lack of English language proficiency contributed to the occurrence of air accidents in the international commercial aviation in the 70s and 90s, discussing, deeply, about the Tenerife disaster. It is characterized as a descriptive bibliographical research, based on previous studies on the subject, regulations and documents. This study described the use of English by both L1 (first language) and L2 (second language) speakers in a bilateral air traffic pilot-controller communication, and how communicative failure can lead to an accident. It has been observed that not only those who have English as a second language, but also pilots and controllers who have English as their mother tongue are subject to communicative failure during their operations. Thus, it is suggested that evaluation tests and training for the use of ACT, implemented in the member countries, consider the idiosyncratic peculiarities of their L1 in order to avoid phenomena such as language code-switching. Moreover, this research concludes by suggesting the need to develop studies aimed at analyzing the behavior of an individual with regard to a lingua franca and his or her tendency to use it in its natural aspect, that is, colloquial and plain English for those who have the English language as a mother tongue, so that more contributions to error-free and safe communication in aviation can be developed.
